**Hospital Link and Family Support Worker -** This is an exciting opportunity to help families and patients who have been affected by acquired brain injury.

We require a reliable, positive, confident team player to fulfil this role. They will be required to be pro-active, trustworthy and a self-motivated individual, with experience of dealing with people in traumatic and emotional situations and ideally, an understanding of acquired brain injury and the consequences.

The role will require you to visit hospitals and liaise with medical professionals, ensuring that brain injured people and their families have the correct information, advice, and support at the earliest stages. To support & signpost families/brain injured people to the relevant Headway services and legal firms, providing crucial support at the acute stage whilst they are at hospital, laying a pathway of further support they will need at the hospital stage and beyond.

The role will require you to provide an assessment of the family’s needs following discharge from hospital. You will be required to follow up on these assessments, helping the family move forward with their lives again by giving information about acquired brain injury, offering emotional support, and completing carer assessments. You will be able to refer to other internal Headway Birmingham & Solihull services and support and signpost externally.
- Provide emotional support as well as advice and guidance for brain injured people and their families / carers, including signposting.
- To provide an assessment of the family and the brain injury needs, thus helping the families and brain injured people to move forward with their lives again.
- Work closely with your immediate team and other internal departments.
- Set up regular ward visits, building and maintaining links with the lead clinicians, physiotherapists, occupational therapists, and other medical team members on behalf of Headway Birmingham & Solihull
- Visit brain Injured people and their carers/family members in hospital trauma wards in Birmingham.
- Meet with families to give support in areas of finance such as PIP completion, benefits, and other areas of support dependant on their needs.
- Signpost to legal firms that specialise in ABI and where required, set up and support families at meetings with solicitors where required.
- Develop working relationships and networking to reach as many people at the early stages of BI/caring as possible.
- Produce required documents for the role and meet deadlines for outcomes, reports, and case studies, as well as personal monitoring items accurately and on time. i.e., end of month, expenses, outputs, and timesheets
- Have existing outstanding working knowledge of Microsoft office packages including Word, Excel and PowerPoint and have excellent communication and written skills.
- **Travel across all areas of Birmingham and Solihull as required. Use of own vehicle, full driving licence is a prerequisite of this role.**
