Care Coordinator

**Semper Care **is an established domiciliary care provider based in Bradford and surrounding areas. We pride ourselves in enabling our service users to maintain their independence as much as possible as well as investing in the personal and professional development of our staff.
This role is being advertised with an option to work 4 days or 5 days during the weekend, including the biweekly weekend on call.
**Job responsibilities**:
- Organise the care services provided by The Organisation and allocate care workers to clients, responsible for all data input relating to specified area as changes occur.
- Allocate staff to services, produce staff rotas on a weekly basis and ensure that they are completed on time.
- Ensure clients have regular care staff undertaking their care.
- Answer telephone calls from clients and staff and deal with their queries in line with the policies and procedures of The Organisation
- Update client and carer records on an ongoing basis and ensure accuracy of records.
- Contact clients and staff and communicate necessary information to them regarding the provision of care services.
- Ensure that the policies and procedures of the company are followed by all staff and take appropriate action where necessary in line with the policies and procedures of the company. Ensure that any inconsistencies with the policies and procedures of the company are reported to the registered manager.
- Ensure a constant bank of carers is available.; place job adverts, liaise with job centres, inform admin/finance staff of new hires, ensure required documentation is processed.
- Taking on new client referrals from social services, private individuals and other organisations
- Ensure holiday/sickness and emergency calls are assigned
- Manage staff holidays and days off.
- Track staff sickness and holidays taken
- Log all compliments and complaints.
- Keep a record of client reviews and manage the client review process.
- Bring to the attention of the registered Manager any complaint made about the company or any of its employees.
- Provide out of hours on-call cover in conjunction with other care staff.
- Attend management meetings as required and contribute to the review of the policies and procedures of the company in order to continually improve the care service provided.
**Essential Requirements**:
- Ability to remain calm and manage situations effectively in any time of crisis.
- Continuity in carrying out good practice.
- Flexible and adaptable.
- Driving license and use of a vehicle for work (with business insurance).
- Communicating in a friendly inpatient manner.
- Experience working with vulnerable individuals with autism
- Minimum 3 years home/domiciliary care experience
- Minimum 1 year care coordinator experience
**Desirable Requirements**:
- NVQ level 3 + in health and social care.
- Training and experience in working with individuals with challenging behaviour.
- Counselling skills.
