Care Coordinator

An opportunity has arisen for a Band 5 or 6 Care Coordinator to work in our Integrated Outreach Team based in Bradford. Successful applicants will be able to demonstrate a high level of commitment, motivation, and knowledge of working with people with severe and enduring mental health, primarily psychosis and often co existing substance misuse. You will need to demonstrate clinically effective, creative and assertive care management skills and interventions. You will have a good awareness of current health and social care issues which affect, or influence, our community. You will be determined to deliver trauma informed care and champion people’s rights whenever necessary. You will need excellent time management skills and, due to the community element of this role, hold a full and valid driving licence. The care coordinator will ensure community-based care to clients with complex mental health needs. They will work within a strong team-based approach simultaneously managing an individual caseload. Compared with generic community mental health teams, care coordinators at IOT have lower caseload sizes. This is necessary to support relatively higher levels of involvement and complexity. Service users within the Integrated Outreach Team are typically people who need a determined, assertive, and creative approach. Their primary problem is a vulnerability to psychosis; many will also experience difficulties related to co-existing problematic substance misuse. The post holder must be recovery focused, working to support enablement and always optimistic about every persons scope for positive change. The post holder will provide high standards of skilled mental health care to our service users and their families within a defined geographical area. The post holder will be expected to provide clinical supervision and support to junior members of the team and actively promote the values and policies of the service and the Trust.
