**Care Coordinator**:
Health and Social care qualifications (NVQ 2/3 or above)
- not essential as we can put you on a training programme.

Valid driving licence with access to a car

Be computer literate and have good office skills

Have excellent customer service skills

Be organised and can work calmly under pressure

Have experience of working in various healthcare settings.

Be a people person with good communication skills.

Support with other tasks required to meet the needs of the business

Conduct care plans and support plans

**Responsible for**:

- Allocation of employees to client visits using effective rostering, ensuring continuity of care and considering locations, skills required.
- Communicating regularly with employees, providing guidance and resolving queries in a timely manner.
- Maintaining telephone contact with clients and employees.
- Ensure telephone calls are answered in a timely, polite and professional manner.
- Ensure all paperwork is completed and entered onto relevant systems.
- Support with the delivery of the on-call rota, dependent on the needs of the business. This will include being the designated contact for the out of hours teams during evening/weekends and supporting all aspects of care delivery.
- Support the field team covering client visits to support the business needs when required.
- Be the 1st line of support for employees and client with care related enquiries.
- Ensure compliance is met within the office and the care calls.
- Assist and support the Registered Manager
- Maintain integrity when dealing with confidential matters.
- Work as part of a team that revolves around person-centred care and support.
