Are you looking for an exciting opportunity to develop your skills in a Community Cardiac Rehab team providing comprehensive cardiac rehab groups around Norfolk and a home based program?

Our team works to support the delivery of Cardiac Rehabilitation (exercise and cardiac health education) for people who have a primary cardiac diagnosis / including myocardial infarction, coronary artery bypass graft surgery and acute coronary syndrome.
- Comprehensive assessment of people referred to the cardiac

rehabilitation service to determine their baseline and discharge clinical outcome measures within a variety of settings.
- Plan and deliver an individual specialist cardiac treatment program using knowledge and evidence-based practice.
- Assist the team to plan and deliver cardiac rehabilitation education sessions and exercise programs.
- Provide specialist advice and education regarding the service to other

professionals within health, social care and the voluntary sector.
- Evaluate the progression of a patient's problems and goal achievement with respect to the treatment plan and adjustment of the management plan as necessary.
- Assist with the triage of referrals to the cardiac rehabilitation service effectively and efficiently to ensure the minimum of delay in meeting the patient's needs.

The team is a multidisciplinary team based in Dereham who travel to different community venues around Norfolk to provide high quality care to our patients.

The team consists of nurses, a paramedic, support worker and a technical instructor who work closely together for the best interests of their patients.
- You will undertake comprehensive assessment of people referred to the cardiac rehabilitation service to determine their baseline and discharge clinical outcome measures within a variety of settings.
- You will plan and deliver an individual specialist cardiac treatment program using knowledge and evidence-based practice and treatment options. The treatment plan is to be based on effective intervention and patient agreed goals taking into account their psychosocial needs.
- You will be assisting the team to plan and deliver cardiac rehabilitation education sessions and exercise program.
- You will obtain the informed consent of patients for the interventions delivered by reviewing and reflecting with the patient their understanding of their problems and treatment options.
- You will provide specialist advice and education regarding the service to other professionals within health, social care and the voluntary sector.
- You will evaluate the progression of a patient’s problems and goal achievement with respect to the treatment plan and adjustment of the management plan as necessary.
- You will be assisting with the triage of referrals to the cardiac rehabilitation service effectively and efficiently to ensure the minimum of delay in meeting the patient’s needs.

Clinical
- You will assess, plan, implement and evaluate all aspects of patient care and develop treatment / care plans that may need to be delivered from a range of options.
- Following a holistic assessment and in liaison with the multi-disciplinary team, devise an individual care program for each patient, modifying it as required.
- You will assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
- Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
- You will be required to use tact and persuasive skills in order to ensure active participation in self-management program, in particular where there may be high levels of anxiety and / or depression.
- You will demonstrate a high level of listening and counselling skills and deal sensitively and confidentiality with an individual’s concerns, particularly at times of high levels of distress or anxiety.
- You will be liaising with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinate appropriately.
- You will be required to undertake risk assessments and act upon them appropriately.
- You will Ensure activity data and clinical information is recorded on SystemOne and completed on a daily basis.
- You will demonstrate dexterity and coordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
- You will order equipment and supplies via the electronic ordering systems used with the team.
- You will be ensuring informed consent is obtained prior to initiating interventions.
- You may occasionally be exposed to bodily fluids or infected material and therefore must utilise universal precautions and adhere to infection control policies.
