The Nurse Practitioner will be responsible for delivering high-quality clinical care, including assessment, diagnosis, treatment, referrals and prescribing. You will triage same-day appointment requests, manage long-term conditions within your scope of practice, and contribute to the overall development of the surgery. Main duties of the job Assess, diagnose, treat and manage patients presenting with undifferentiated conditions Perform physical examinations Prescribe medications Order and interpret diagnostic tests Referral to secondary care as appropriate Support chronic disease management Recognise and manage acute deterioration and red flags Work collaboratively with GPs and wider primary care team Participate in audits, QOF and evidence-based practice Maintain accurate clinical documentation Home visits About us Orchard Surgery is a forward thinking GP surgery serving a registered patient population of 13,200. Medical Team: 8 experienced GPs providing a wide range of clinical expertise Advanced Nursing Support: 3 Nurse Practitioners offering extended clinical services Nursing Team: 2 Registered Nurses and 1 Nurse Associate ensuring continuity of care Healthcare Assistants: 3 HCAs supporting patient care and clinical efficiency Pharmacy Team: 5 The Team focuses on optimizing medication therapy, working directly with patients and healthcare teams to improve outcomes and safety. Admin and Care Navigator Team: 15 The team provides essential support within the GP practice, combining administrative efficiency with patient-focused navigation to ensure smooth access to care. Together, this multidisciplinary team works closely to provide comprehensive healthcare services, with a strong emphasis on teamwork, innovation, and professional development. Job responsibilities Key Responsibilities Conduct comprehensive assessments, including history taking, physical examinations, and clinical decision‑making. Diagnose, treat, and manage a range of acute and chronic conditions in accordance with clinical evidence and practice protocols. Provide urgent/on‑the‑day appointments, triage, and manage minor illness and injuries. Initiate and manage medication prescribing within scope of practice (for independent/supplementary prescribers). Interpret diagnostic tests including blood results, ECGs, urine tests, and swabs. Implement health promotion and disease prevention strategies. Prioritise and manage a personal caseload, ensuring timely and safe patient care. Collaborative Practice Work collaboratively with GPs, Practice Nurses, Pharmacists, HCAs, and administrative staff. Contribute to multidisciplinary meetings, case discussions, and clinical audits. Refer patients to community and secondary care services where appropriate. Support the coordination of care plans for patients with complex needs. Professional Development & Governance Maintain professional registration and adhere to the NMC Code of Conduct. Participate in clinical supervision, annual appraisal, and mandatory training. Engage in continuous professional development to enhance clinical skills. Contribute to the development and implementation of clinical policies, procedures, and guidelines. Participate in significant event analyses and quality improvement initiatives. Patient Experience & Safety Ensure a high standard of patient care, empathy, and confidentiality at all times. Promote patient empowerment, self‑care, and health literacy. Manage clinical risk and maintain accurate, timely clinical documentation. Follow infection prevention and control policies. Essential Qualifications & Experience Registered Nurse Practitioner with NMC registration. Masters level qualification or equivalent in Advanced Clinical Practice (or working toward). Independent prescribing qualification (V300) preferred or willingness to obtain. Experience working in primary care, urgent care, or community settings. Strong assessment, diagnostic, and clinical decision‑making skills. Desirable Qualifications & Experience Experience managing long‑term conditions within primary care. Minor illness/Minor injury training. Experience using primary care systems (SystmOne). Evidence of QOF or audit involvement. Key Skills & Attributes Excellent communication and interpersonal skills. Adaptability and resilience in a dynamic clinical environment. Person Specification Qualifications Qualifications & Training Registered Nurse (NMC registration). Evidence of continuing professional development relevant to advanced practice. Minor Illness/Minor Injury training. Physical assessment and clinical examination training (if not part of MSc). Interest in developing specialist clinics or extended roles. Willingness to contribute to practice development and leadership activities. Up‑to‑date mandatory training (e.g., BLS, safeguarding, infection control).
