Clozapine Support Worker

This post will enable you to become a key member of the trust clozapine service where you will be able to help continue to drive forward the excellent patient focussed care already provided.
You will be in direct contact with patients building a regular therapeutic relationship with them as they come for their required blood and physical health tests required as part of their clozapine treatment, that forms part of the ongoing care plan for that individual.
You will work in clinics across the trust in conjunction with other pharmacy team members and under the supervision of the lead nurse for the clozapine service.
The post holder will be responsible for supporting and delivering both clozapine clinic and the physical health care agenda within the trust clozapine service. Linking a variety of physical health care observations and assessments to the on-going work to support the service user with an integrated and comprehensive care package between specialist mental health services and primary care services under the supervision of the lead clozapine technician and clozapine nurse.
They will also support the delivery of the medicine management and optimization agenda, helping to ensure the delivery of an efficient and effective medication supply/returns service to inpatient & community sites across the Trust.
Support the physical health care agenda through the clozapine clinics by taking part in phlebotomy, taking ECGs, taking physical health observations (including but not limited to Blood pressure, weight, height, blood glucose levels, encouraging healthy eating and physical activity advice)
To be able to interact with patients and provide clozapine care plans under the supervision of a Band 6 nurse
Support Band 6 nurse with admin responsibilities including scanning

Support Band 6 nurse with admin responsibilities including scanning.
Assist the pharmacy team in ensuring that any action plans arising from the strategy of medicines management are achieved in a timely fashion, including the undertaking of any pharmacy based audits.
Ensure adherence to all local SOPs and trust wide policies on medicines management.
Support the operational running of the clozapine clinic, including setting up and running the POCHi blood analyser machine, if and when trained to do so.
Delivering medication to patients unable to attend the clozapine clinics in person.
Support the expansion of the clozapine clinics to include patients who are on other medications that need close monitoring (including but not limited to antipsychotics; Lithium; & Valproate).
Support the timely ordering and receipt of named patient supplies for clinics & stock items, including emergency drugs, for sites to pre-determined stock levels.
Be responsible for ensuring that all medication no longer required is recorded and returned via approved returns mechanisms.
Support the provision of information and pharmacy paperwork in the pharmacy folders ensuring that they contain correct and up to date information.
Ensure that the security of medication on the sites is maintained at all times and report any breeches of security to a member of the senior pharmacy team.
Ensure that the storage of medication is maintained and report any concerns or storage failures to a member of the senior pharmacy team.
Highlight any concerns or risks around medicines management to the senior pharmacy
