Caseload Coordinator - some weekend working required

This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.

UCR/VW is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis The service can clinically monitor patients remotely via Virtual Ward/Hospital at home using enhanced technology which is supported by a Digital Health team.

This role will involve managing the case load of clinical staff within the Urgent Community Response (UCR) and Virtual Ward (VW) Team. It will involve gathering, inputting and providing essential data for patients, helping to maintain accurate records and enhancing the team’s ability to provide personal care.

The role is responsible for the administration support and caseload co-ordination within the team to support the delivery of care to patients. The post holder would also be required to answer the telephone, which will include calls from patients, families, carers and other service users, accurate information will need to be recorded and forwarded onto a clinician.

Ensure that the team case load is kept up to date.

To participate and co-operate with the Multi Disciplinary Team to contribute to the effectiveness of the service as a whole.

To manage and maintain the team’s clinical caseload, including all associated administration tasks.

To use multiple patient information management systems (EMIS, Docobo, Patient Centre etc.) to enter such information as required on the computerised patient information system.

To use discretion, sensitivity and deal with information in a confidential and professional manner at all times.

Oversee and monitor the distribution/collection of equipment that is used to monitor patients in their own home (tech-enabled care).

Maintain an accurate database of the home monitoring devices.

To be responsible for data collection, based on the outcomes from the clinical caseload.

To participate in and organise team audits when appropriate.

If successful this vacancy will require an standard DBS check at a cost of £21.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
