We have an exciting opportunity for a Community Co-Ordinator (Apprentice and Care Navigation) to join our inspiring and friendly team at our Care Navigation services.

Your role will be a field based position where you will work flexibly whilst covering the geographical area of North East with our sites being based between Northumberland and Yarm. You will work full time, 35 hours per week on a permanent contract with a salary of £25,690.03 to £28,879.59 per annum. Our Care Navigation services aim to provide a seamless package of support to individuals leaving secure settings and returning to the community, reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending.

How will you make a difference?

**As a Community Co-Ordinator you will**:
provide leadership support and supervision to Care Navigators and Apprentices

work closely with Senior Management to provide a high quality, patient centered service.

support the Community Lead (Criminal Justice and Secure Care) with meeting all demands and objectives relevant to the contract

support the co-ordination and facilitation of services and the Durham Reconnect Hub

provide Care Navigation and Apprentices support

take a lead role in service user engagement, including facilitation of service user forums and support of peer mentors

support mental health promotion

be able to work autonomously and from a number of locations, travelling independently for the role as needed

be able to establish and maintain effective relationships with a range of professionals using excellent interpersonal skills

assess new referrals for suitability based on service criteria

attend multi-disciplinary meetings relating to referrals or clients in treatment.

work closely with other members of the team ensuring appropriate care and management is provided

ensure that client confidentiality is protected at all times.

record and evaluate 'outcome measures' (SWEMWBS) as part of the ongoing therapy process

ensure the accurate entry of clinical data onto agreed systems, maintaining the highest standards of clinical record keeping.

work in partnership with other disciplines and to maintain links with statutory and non-statutory agencies as appropriate, in relation to continuing care on release from prison.

adopt a flexible and pragmatic approach to ensure the highest standard of service delivery is provided.

adhere to an agreed activity contract relating to the number of client contacts offered, and the length of care provided to each patient

support the collation of monthly performance data for service management team.

will communicate effectively with patients, stakeholders, colleagues, and relevant professionals, both in verbal and written form

be able to work autonomously and manage my time and work effectively, balancing the needs of the client and the organisation effectively

What we are looking for?

**You will**:
have QCF / NVQ level 3 in Health & Social Care, or formal qualification relevant to the prison based Mental Health Resettlement Officer field

hold a full UK driving license and have access to my own transport

have a working knowledge of debts, housing, mental health, social care, criminal justice and/or substance misuses

have relevant experience in a comparable role

be experienced in maintaining professional boundaries, even in challenging situations

have the ability to make a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans to meet these needs.

be flexible with my work and am able to use my own transport to travel regionally for business purposes

have the ability to develop good rapport with service users and practice in a non-discriminatory manner, respecting the beliefs and cultural practices of individuals and groups.

operate at all times from an inclusive values base which promotes recovery and recognises and respects diversity.

have xperience of using clinical outcome measures to inform practice.

be a confident communicator with excellent verbal and written skills.

have working knowledge of standard IT packages including Microsoft Word, Excel and use of database / information systems.

have ability to use line management and clinical supervision and personal development positively and be self-reflective in own personal and professional development.

be able to prioritise workload demands and positively respond to unforeseen challenges.

deal with stressful and difficult situations in a calm manner.

You may also have

Experience working in prison environment or experience of working with offenders.
