The purpose of this role is to provide high quality holistic support to older people to allow them to remain living independently, safely and well at home. Working closely with GP surgeries, Multidisciplinary Teams and Frailty Teams across Newport, you will take a holistic approach to support.

Support will be person centred and will be based on a ‘Stay Well Plan’ which focuses on all aspects of their daily life. The plan will help identify areas where support may be required, and will allow you to actively promote a client’s wellbeing as well as signpost to any internal and external relevant services to create resilience and to support them to remain living independently in their own home.

You will manage your work time and case load effectively, have a passion for supporting older people and be flexible in order to meet service needs. **The ability to drive and have access to of your own vehicle for work purposes is essential to your role. You will also undergo an enhanced DBS check.**

**Main duties and responsibilities**
- To have a clear understanding of the needs facing older people and providing quality support, creating resilience and independence at home
- To assess the needs of clients identified by GP’s (Risk Stratification Tool) and the Frailty lists and draw up a Stay Well Plan reflecting their personal requirements
- To deliver outcome focused service provision within the community, regularly updating GP surgeries
- To assess the needs of the client and draw up a Stay Well Plan reflecting their personal requirements
- To visit or contact according to the client’s changing needs and provide emotional and practical support and encouragement
- To provide information, options, assistance and signposting on community services, practical aids and adaptations
- To attend regular multi-disciplinary meetings within GP settings (within the community)
- To act in a non-judgmental way
- To log clear and concise case notes on IT systems (Charity Log) ensuring these are up to date and working in line with Organisational standards
- To maintain and compile detailed records to ensure accurate statistics can be created as required by the funders
- To maintain confidentiality and understand data protection - GDPR regulations
- To provide quality support to clients in the community, from an office, via telephone, Teams, Zoom or WhatsApp as directed by the client
- To ensure that safeguarding of clients is a key priority and take any necessary internal and external procedures and action
- To form an active part of the Healthcare Team and attend team meetings, in[1]dividual meetings, external meetings, supervisions and annual appraisals
- To actively promote the service, attending internal and external meetings as required
- To maintain accurate working week schedules via Outlook to claim expenses, and book holidays online (iTrent)
- To perform additional routine administrative as directed by the Department Manager when required
- To carry out any additional duties as required by the Department Manager
- To be prepared to undertake regular training as required
- To be willing to travel and work flexibly throughout the charities operational area
- To be prepared to work from any of our operational bases as required

**Person Specifications**

**Education and Training**
- Experience in a similar role or a qualification in a relevant discipline or a willingness to undertake such a qualification
- A well-developed knowledge of IT either by qualification or experience in using databases, Outlook, and Microsoft Office
- Admin/organisational skills and experience
- Driving license and access to a vehicle for work purposes

**Skills and Aptitude**
- Experience of working with older people
- Good communication and interpersonal skills
- Knowledge of data protection - GDPR
- Able to follow routine administration procedures
- Able to work as part of a team
- Ability to use initiative and work independently
- Experience of assessing and maintaining a caseload
- Full IT Literacy; the role will involve daily use of online databases, Outlook and Microsoft Office
- Willingness to work flexibly between the hours of 9 - 5pm Monday to Friday to meet the needs of the business (some occasional weekend work may be required)
- Willing to undertake any relevant training

**Desirable Criteria**
- Previous experience of engaging with the older people
- An understanding of working within or with a GP Practice
- Experience of developing and maintaining a support/care plan
