Overview
We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. The successful candidate will play a vital role in organising and coordinating patient care plans, ensuring seamless communication between patients, families, and healthcare professionals. This paid position offers an opportunity to contribute meaningfully to patient well-being while developing valuable organisational skills. Prior office experience is preferred but not essential, as training will be provided.
Responsibilities
- Act as the primary point of contact for patients and their families, providing support and guidance throughout their care journey
- Coordinate appointments, treatments, and follow-up schedules in collaboration with healthcare providers
- Maintain accurate and up-to-date patient records using office management systems
- Facilitate communication between multidisciplinary teams to ensure cohesive care delivery
- Assist with administrative tasks such as data entry, filing, and correspondence related to patient care
- Organise resources and educational materials for patients to promote understanding of their health conditions
- Support the implementation of care plans by monitoring progress and addressing any concerns or barriers
Experience
- Previous office experience is advantageous, demonstrating organisational skills and familiarity with administrative procedures
- Strong communication skills, both written and verbal, with a compassionate approach to patient interaction
- Ability to prioritise tasks effectively in a fast-paced environment
- Proficiency in basic computer applications such as MS Office (Word, Excel, Outlook)
- A genuine interest in healthcare and supporting patient well-being is essential
