1.JobSummary The Senior Care Coordinator will be the first point of contact for patientsand service users providing an in-depth knowledge of practice and local community services. The Senior Care Coordinator will liaise closely with their clinical team andmanagementteamstoensurepatientsreceivetimelyandappropriate direction or appointmentsaccordingtotheir healthcareneed. There may be a need to provide cover to the general administrative team within their practice. 2.KeyRelationships The post holder will have regular contact with a wide range of clinical andmanagerial staff, administrative support staff and local health/social care service providers and charities.
3.DutiesandResponsibilities The duties and responsibilities listed below are representative of the Senior PatientCare Coordinator role and its purpose within the network Practice. MainDuties Working within the leadership team to; Manage and support the reception team Manage premises, facilities and equipment Ensure CQC compliant Ensure Health and Safety compliance (e.g. calibration, fire drills and monitoring, risk assessment etc) Rota Plan Coordinate seasonal flu/covid vaccinations Monitor and follow up QOF targets Develop care navigation systems and improve patient access Project manage as required based on changing annual targets (e.g. SMI Health checks developing and setting up services, taking the lead in setting such projects up etc) Undertakeanytrainingasrequiredtocompletethetasksassociatedwiththejob role.
Escalate any issues where appropriate to the Practice Manager. This list is not exhaustive and roles may be added or changed to meet the needs of the business Confidentiality/DataProtection/Freedomof Information Post holders must maintain the confidentiality of information about patients,staff and other health service business in accordance with the Data ProtectionAct of 2018. Post holders must not, without prior permission, disclose anyinformationregardingpatientsorstaff.Ifanymemberofstaffhascommunicated any such information to an unauthorised person they willbe liable to dismissal.Moreover, the Data Protection Act 2018 also rendersan individual liable for prosecution in the event of unauthorised disclosure ofinformation. Following the Freedom of Information Act (FOI) 2005, post holders must applythePracticesFOIprocedureiftheyreceiveawritten requestforinformation.
RiskManagement All post holders have a responsibility to report risks such as clinical and non-clinical accidents or incidents promptly. They are expected to be familiar withthe Practices use of risk assessments to predict and control risk, as well as theincidentreportingsystem forlearningfrommistakesandnearmissesinorder to improve services. Post holders must also attend training identified by theirmanager,orstatedby thePracticetobemandatory. FlexibleWorking As an organisation we are committed to developing our services in ways thatbest suit the needs of our patients.
This means that some staff may be asked to work a more flexible shift pattern so that we can offerservicesin theeveningsorat weekends. Safeguardingchildrenandvulnerableadults Postholdershaveageneralresponsibilityforsafeguardingchildrenandvulnerable adults in the course of their daily duties and for ensuring that theyareaware ofthe specific dutiesrelating totheirrole. They must attend mandatory training for Safeguarding. SmokingPolicy The Practice is committed to providing a healthy and safe environment for staff,patients and visitors.
Staff are therefore not permitted to smoke on Practiceproperty.
