Chesterfield is a busy, thriving and sought after place for families to live and work in. The successful applicant must hold the Specialist Public Health Nurse and Specialist Practice Teacher (Health Visitor) qualifications.

They will work within a caring, professional and high functioning Health Visiting Team. You will support, teach, and develop programmes of learning experience for specialist practitioners/students and new staff within the 0-19 service and assist in the formal assessment and competence to practice at that level to meet the requirements of the NMC, Higher Education Institution and Derbyshire Community Health Services.

You will also assist in the collation, delivery and presentation of statistical information and data as required to meet the NHS and local children’s services agendas.

You will be based with one of the two Health Visiting Teams in Chesterfield and have regular contact with all the Practice Educators across Derbyshire to support you in delivering all aspects of your role.

The teams within Chesterfield are supportive towards each other and foster positive team behaviours. You will be expected to demonstrate leadership and exemplary practice within the team.

You will provide clinical leadership to support individual professional development and practice development. You will be a role model for excellence in teaching, supporting learning and quality service delivery.

The role will entail promoting clinical governance guidelines and be proactive in the development of a culture that is committed to innovation and quality improvement using research and audit to enable services to be continuously improved, to include clinical benchmarking, professional development, competence frameworks and clinical supervision.

You will also manage and take continuing responsibility for a defined caseload/geographical area and be responsible for the assessment of care needs, the development, implementation, and evaluation of programmes of care and setting of defined standards of care which are evidenced within the Standard Operating Procedures (SOP) for 0-19 service and meet Trust Policies and Procedures to ensure quality of care for patients.

You be responsible for the development and identification of learning needs and ensure - a positive culture for learning to support the development of Specialist Public Health Practitioners, Skill Mix, and students (Pre and post reg). Your role will also support the senior management team by attending national and local events to support the development of the 0-19 children’s services within DCHSFT.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
