**Brompton Health Care Coordinator**
Do you have a specialist interest in tackling health inequalities, and improving specific health priorities? This is an exciting opportunity to work within our diverse Brompton Health Primary Care Network consisting of 11 GP practices, to support in the effective uptake of childhood immunisations. You will also have the opportunity to play a part in other health priorities including hypertension, cancer screening and more. Your role will be to focus on engaging with patients at risk of health inequalities, and using your skillsets to improve their health outcomes, by encouraging them to participate in public health interventions. You will work with a diverse range of people from different cultural and social backgrounds.

**About Brompton Health PCN**:
Primary Care Networks (PCNS) enable GP practices to work together in a collaborative way to develop and deliver network-based services that respond to the needs of the local population. Brompton PCN covers a varied population demographic. The PCN works through a set of guiding principles which includes working for the greater good of its population and patients, a mission to improve services, be innovative, and continue to improve and deliver excellent NHS services uniformly across all practices.
**Post Title: Care Coordinator**
**Accountable to: Chief of Operations LMA GP Federation**
**Responsible to**:Brompton Health Clinical Director (Dr Anna Cantlay) and Federation Development Manager Clinical Lead for allocated priority area (Dr Ben Brodie for Hypertension and Early Cancers) and (X Practice nurse for childhood immunisations)
**Key Relationships**:Patients
Host Practice Site
General Practitioners and other staff in GP surgeries
PCN Primary Care team members
**Hours**:Up to 37 hours per week. There is also the opportunity to work part time pro rata a minimum of the contracted hours each week will be 21 hours within this period and the days/hours to be worked will be agreed weekly in advance with the employee
**Salary**: £27,000 per annum
**Contract**:This is an initial 6-month contract with the possibility to review
**Purpose of the Role**
A care coordinator will be allocated to focus on specific population health priorities across the primary care network. There will be two care coordinators working on a priority area in a buddy-system, working across all 11 practices within the PCN. Specific training and Standard Operating Procedures relating to the allocated area will be provided. A care coordinator will;
- Utilise population health data to proactively identify and work with a cohort of patients, delivering personalised care and improving outcomes in the following key priority areas:
- **Hypertension**:
- Improving early cancer diagnosis - cervical screening, bowel, breast and prostate screening
- Uptake of childhood immunisations
- Particularly focusing on harder to engage patients and those at risk of health inequality using population health data metrics.
- Have an awareness of the key contractual care processes and key performance indicators relating to the priority areas, including QOF, Impact and Investment Fund and Out of Hospital Enhanced Services.
- Utilise data analytic software and tools to identify and liaise with those who need reviews and appointments and facilitate the booking of these as needed. This may include**:Running Clinical Audits and Searches**:
- Identifying, calling and supporting patients who are eligible for care as part of local or national specifications to improve healthcare for the individual practice and PCN population level.
- Supporting patients in the booking of appointments at practices such as blood tests, Blood pressure reviews, immunisations and smear tests.
- Support in developing new initiatives to improve the uptake of health checks and screening for priority cohorts via various patient engagement activities.
- Support patients to utilise decision aids in preparation for a shared decision-making conversation, providing them with evidenced-based information to allow them to make an informed choice.
- Holistically bring together all of a person’s identified care and support needs, and explore options
