About the Role
We are seeking a compassionate, organised, and highly motivated PCN Coordinator / Administrator to support the effective delivery of patient care across our Primary Care Network. You will play a vital role in coordinating the work of healthcare professionals and non-clinical staff, ensuring patients receives seamless, personalised, and high-quality care.
As a key member of a dynamic, forward-thinking multidisciplinary team—working across wider PCNs, Community Services, and Local Authorities—you will help deliver enhanced support to diverse groups of patients.
The ideal candidate will bring excellent administrative abilities, strong communication skills, and a caring, patient-centred approach. You will often be the first point of contact for our patients, so the ability to listen, reassure, and communicate clearly is essential.
Key Responsibilities:
· Work with patients, families, and carers to support their understanding of health conditions.
· Assist patients in developing and reviewing personalised care and support plans.
· Provide appointment management, respond to queries, and ensure patients receive accurate written and verbal information to support informed decision-making.
· Support patients in accessing self-management education, peer support, health coaching, and wellbeing interventions.
· Provide coordination and navigation across health and care services to ensure patients receive timely, coordinated support from the appropriate professional.
· Collaborate with GPs and other primary care professionals to proactively identify and manage a caseload of patients, including those with long-term conditions.
· Refer patients to appropriate PCN healthcare professionals when required.
· Support the coordination and delivery of multidisciplinary team activities within the PCN.
· Raise awareness of shared decision-making and support both staff and patients to participate effectively in these conversations.
Skills & Experience Required
· Strong administrative experience within a busy environment.
· Excellent communication skills with the ability to demonstrate empathy and put patients at ease.
· Ability to multitask prioritises workloads, and work well under pressure.
· High attention to detail and accuracy in data handling and documentation.
· Strong IT skills, including proficiency in Microsoft Office, Excel, PowerPoint, and Microsoft Teams.
· Ability to analyse data and produce clear, concise reports.
· A collaborative team player with a proactive and supportive approach.
