Diabetes Pcn Coordinator

The post holder will provide a high quality administrative and secretarial support to the Harrow Community Diabetes and Primary Care Networks (PCNs). Demonstrating a high level of communication and interpersonal skills in dealing with a wide range of NHS staff, patients, and external agencies.
A substantive initiative is required in providing support to CLCH Community Diabetes Team, Primary Care Network and representative from other organisations.
The post holder will establish and co-ordinate PCN MDT activities, liaising with GPs, senior management, patients, clinicians to ensure smooth running of the Community Diabetes Service and ensuring that all activities are recorded on EMIS record system and duties performed efficiently and within agreed and defined timescales.

- To provide comprehensive high quality secretarial and administrative support to the Primary Care Network team and Community Diabetes service.
- To utilise project management skills to ensure all components of the PCN agenda is in place according to agreed timescales.
- To liaise with Nurse Consultant, team leaders to ensure smooth running of the PCN MDT and activities
- To be responsible for marketing and promoting the Community Diabetes Service and PCN work. This will include developing promotional material, meeting GP’s, Practice nurses and other health professionals to discuss PCN programmes, promoting the programme to wider community services.
