**About The Role**:
Two positions are available for the role of Assessment Support Coordinator in North Dorset, based in Sherborne, Shaftesbury or Blandford.

As an Assessment Support Coordinator, you will be working alongside Social Workers, Occupational Therapists, and health colleagues across a range of settings to support people to remain as independent as possible and linked in with their local networks.
- you will be undertaking strengths-based assessments and support planning, in accordance with the Care Act 2014
- you will assist individuals to identify their goals and how best to achieve them to support their independence, and where appropriate complete mental capacity assessments
- you will be working as part of a busy team and alongside other teams within the local authority. Multi-disciplinary practice will be integral to the role

**About the Team**:
North Dorset is the most rural part of Dorset and 29% of the population is older people. The Adult Locality Team works in various co-located hubs with health professionals across the patch in Blandford, Sherborne, Shaftesbury and surrounding villages. The North Locality Team has a strong track record in supporting new starters to Dorset Council Adult Services by being friendly and supportive, developing individuals and detailed and comprehensive induction plan.

These are full time roles at 37 hours per week.

This post requires significant travel. You must have a vehicle (or transport we deem suitable) available for use as needed.
