An exciting new role has been created within the dynamic and well-established Adult Safeguarding and Learning Disability (all ages) Team. The successful individual will work alongside the Adult Safeguarding and Learning Disability Deputy Lead Nurse in supporting vulnerable adults, their families & the staff who care for them. You will be based mainly at Chelsea and Westminster Hospital but deputising cross-site as required.
- Supporting vulnerable patients & their families during admission, outpatient appointments, preparing for hospital visits and for discharge.
- Providing training for staff
- Liaising with hospital teams, community healthcare & social care services regarding the care/ needs of patients
- Participating in the development of policies & strategy to provide safe, effective care for vulnerable patients
- To maintain professional boundaries and working relationship with patients and colleagues
- Effectively escalate to Deputy Lead Nurse any situation that may be detrimental to the health

and wellbeing of an individual / carer/ family with a learning disability and/or autism
- Ensure confidential information is imparted sensitively and accurately to medical staff,

Chelsea and Westminster Hospital NHS Foundation Trust provide services from two main hospitals, Chelsea and Westminster Hospital and West Middlesex University Hospital, and a number of clinics across London and the South-East.

We have nearly 6,000 members of staff that care for nearly one million people locally, regionally, nationally and internationally. Both hospitals provide full clinical services, including full maternity, emergency and children’s, in addition to a range of community-based services across London, such as award-winning sexual health and HIV clinics.

We are one of the best performing FTs in London for A&E waiting times, and mortality rates are better than average. Since the Care Quality Commission last inspected our hospitals we have taken on responsibility for a range of new services. Our new services include:

- Cardiac catheter service and a paediatric assessment unit on the West Middlesex site
- Surgical assessment unit on the Chelsea site
- New sexual health clinic at 10 Hammersmith Broadway
- Virtual fracture clinics
- Learning disabilities passports

Our facilities are some of the best in the country. We have been investing around £10 million a year in our estate. We are currently spending c. £25 million on expanding our adult and neonatal critical care facilities at Chelsea and Westminster and redevelopment of our children’s unit at West Middlesex - in partnership with our charity, CW+ and generous donors.

Clinical Role
- Work in partnership with Trust and community services to support appropriate notification of patient (people with learning disability/ autistic people / at risk of abuse) to safeguarding team to support review and oversight of care management as appropriate
- Actively engage with Trust staff to assist and empower them to provide safe and equitable care for people with Learning Disability and autistic people and adults experiencing or at risk of abuse
- To work creatively with the Trust staff in planning and supporting a safe pathway for people with a Learning Disability and autistic people whilst accessing acute services advising on making reasonable adjustment to care services Management Role

Management Role
- With the Safeguarding Project officer, contribute to the Learning Disability and Adult Safeguarding quarterly and annual Reports
- Identify and make recommendations for improvements in standards of service delivery for the enhancement of care of people with Learning Disability and autistic people and adults who are at experiencing or at risk of abuse
- Work in partnership with the Community Learning Disability Services and Adult safeguarding services including Education, Social Care and Health
- To escalate to their line manager any clinical and professional concerns that may affect the service including capacity, demand and quality in a timely fashion
- To meet with their Line Manager on a regular basis for clinical supervision.Deputise and act up for the Deputy Lead Nurse in their absence
- Participate in meetings and project work as identified and delegated by the Deputy Lead Nurse
