Velindre Cancer Centre have been working in partnership with Macmillan to develop this exciting Macmillan cancer support navigator service. The service will provide a central point of contact for all cancer patients and the post holders will work alongside the clinical nurse specialist team to enhance patient care and communication.

The post holder will act as a central point of contact for patients and triage calls to the CNS team escalating clinically important calls. The role will provide support, signposting and advice to patients on non-clinical enquiries and provide guidance, contact and support for patients as they journey through our hospital(s). The post holder will provide guidance and information to ultimately improve patient experience.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

This post is fixed term for 3 years due to - meet the needs of the service
- Develop communication skills in order to convey complex and sensitive information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events
- Communicate with people in a form and manner that is consistent with their level of understanding and culture, and recognise barriers to effective communication
- Communicate clearly, effectively and appropriately with the

multidisciplinary team, patients and their family/visitors/carers
- Respond appropriately to queries, take phone messages and pass on

written and verbal information to patients
- Provide relevant information to patients, family/visitors/carers and colleagues
- Complete documentation accurately and contribute to reports of patients' activity and progress, recognising that the CNS has personal accountability for entries to patient records made by others under their supervision
- Provide feedback on patient care and/or relevant patient communications to relevant healthcare professionals
- Participate in discussions about patient care and clinical service

development, including relevant team meetings
- Maintain patient confidentiality
- Support worker experience
- NVQ 4 or 3 plus additional training to diploma level equivalent or relevant experience
- To have knowledge of cancer related issues
- Experience of team working
