In partnership with Macmillan Cancer Support, we are delighted to be recruiting for Macmillan Cancer Support Workers. These are new roles within our Cancer Services department and will be for a fixed term period until September 2025.
This is an exciting opportunity to directly support people on the suspected cancer pathway, helping them navigate the health and social care system. Under the supervision and delegation of cancer Clinical Nurse Specialists, the Macmillan Cancer Support Workers will be integral members of the cancer key worker team. They will work closely with a wide range of health professionals and partners to provide and facilitate financial, practical, emotional and physical support for people with cancer.
By undertaking the Macmillan Electronic Holistic Needs Assessment, the Macmillan Cancer Support Workers will have supported conversations with people about what matters to them and will co-produce a care plan supporting people with the non-clinical aspects of their care. They will enable people to self-manage by providing relevant information and advice, and signposting to other resources and services available.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
An integral member of the cancer Key Worker team, working closely with cancer Clinical Nurse Specialists and cancer multidisciplinary teams.
A key aspect of the role involves direct communication (e.g. face to face and virtual (telephone and video) with patients. The information and nature of the communication required is sensitive due to the nature of cancer. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.
Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and pathways. To use own judgement to assess complexity of enquiries and refer to manager, to clinical teams and to other organisations as appropriate. Provide basic telephone advice and refer on or sign post to other sources of support.
To set up and undertake a Macmillan Electronic Holistic Needs Assessment to inform a supported conversation with people about their non-clinical concerns. The Macmillan Cancer Support Workers will be central to the delivery of excellent person-centred cancer care, having a significant impact on every part of the cancer pathway and supporting people with cancer when they most need it.
Work with cancer MDT co-ordinators, cancer trackers and clinical teams to facilitate and support peoples transition along the pathway.
Document information in a legible, accurate and professional manner.
Computer literate with experience in use of database, presentations, data entry and retrieval.
