Katherine Harriet Bespoke Home Care are seeking a motivated and committed individual to join our Office Team as a Client Team Officer. This role is Monday - Friday 08.45am - 17.15pm and will be required to be part of the On Call team.

**Week 1 - Client Team Administration / RM Support**
- To ensure that all Client paperwork is prepared and ready to be taken to Client meetings
- Ensure that all Client Assessments and Reviews are booked and attended, paperwork is completed to an outstanding standard and signed
- Ensuring that before any care support commences, a client’s assessment and ALL risk assessments have been signed off by RM
- Ensure that all Client Care Plans are completed to house standard, and person-centred
- Ensure that the Client is provided with information about the service so they are clear on what to expect and how they can raise concerns, in line with KH procedures and as directed by the Registered Manager
- Ensuring new Clients Forms / Uplift Forms are completed for new and existing Clients and loaded onto the system effectively and promptly
- Ensure excellent communication with Clients, their families and representatives, staff, and other health and social care professionals to deliver high-quality care services
- Ensure that all Reviews and On-Boarding processes are kept up to date and booked in for the Client Team
- Ensuring all meeting preparations are carried out before the meetings and notes are typed up to our in-house standards and distributed to the accurate individuals on time
- To complete a post-it for any QA and Incidents that may arise
- To support with any medication ordering for clients
- Any other Care and Operations administrative duties as outlined by the Management team
- Assist with weekly social media update from the Client Team to be completed by 12pm latest every Wednesday

**Week 2 - Templating and Rostering Duties**
- To ensure the templates are completed for week 2 rotas, deadline 5.00pm every Tuesday to 85% every week
- Coordinate and manage the compilation of weekly rotas (Week 2), to be completed by 12.00 pm every Wednesday and all calls are allocated
- To ensure that the unallocated is completed and cleared daily before leaving for the day and on a Friday ensuring that this is cleared until Monday lunchtime
- To complete WBA holidays each week and communicate the approvals and declines to them, by 5.00pm every Thursday
- Support the Mentor Team with regular communication and updates
- To support the Staff team with new availability form and the approval of these, ensuring that they are correctly entered onto the system
- To liaise with the Client Team to ensure that the onboarding calls are carried out each week by 5.00pm every Thursday
- Manage Top 10 Clients with the RM and update RAG List weekly
- To attend all Client Team Meetings to discuss any rota issues and resolve these
- To support the staff team with shadowing of new WBA and ensure that the paperwork for this is filled out and all communications to clients and staff are made
- Any other Care and Operations administrative duties as outlined by the Management team.

**Salary**: From £24,000.00 per year
