As this is a new role, the duties and responsibilities of the Cancer Care Co-ordinator will evolve and develop over time and will include but are not restricted to: To assist the Primary Care Network (PCN) in delivering improvements to the services we provide our patients in relation to patient participation in national cancer screening programmes. Ensuring robust and supportive referral practices are in place for patients suspected of having cancer; including use of guidelines, professional development, and safety netting of those referred. In support of collaborative working the post holder will form productive and supportive relationships with practice and PCN staff who have the skills, knowledge, and remit to contribute to this work. e.g., GP practice nonclinical cancer champions, social prescribers, pharmacists, medical secretaries etc The post holder will assist the PCN member practices to evaluate their screening uptake and work to engage with hard to reach populations and to reduce health inequalities.

Including appropriate contributors from other organisations. Source appropriate resources, training, and system examples from appropriate organisations such as the Cancer Alliance, Cancer Research UK, Macmillan, and other local authority partners. Be a point of contact for PCN member practices to assist them in developing and implementing their cancer screening improvement action plans Build relationships with community assets and develop a service directory of support for the patients within our PCN and a Library of PCN data packs and other resources to support the delivery of information available to patients in a wide variety of formats to meet the needs of all patient groups, including those with learning disabilities, and those people for whom English is not a first language. Review practice coding for report building and templates to ensure consistency across the PCN and accuracy of data.

Identify coding anomalies and liaise with Ardens (template and reports used by all member practices). Be responsible for direct clinical data inputting onto the EMIS system for patient contacts. Keeping up to date with National/Local Cancer Strategies. To be accountable for ensuring that all clinical records, including work diary, are maintained in accordance with the practices records management and Information Governance policies.

To be accountable for ensuring personal compliance with all General Practices policies and procedures including safeguarding children and adults and health and safety, alerting the line manager of any issues or concerns in relation to delivery of the service. To attend monthly PCN meetings and produce performance and quality improvement reports for the PCN as and when required. Work with the practices by providing an advocacy role of support to those identified as needing support who are referred for a 2 week referral.
