**Overview**

Due to the nature of the business, this role will be office based 2 days a week, with additional hours available supporting staff and individuals on the community.

Hours available between 12 - 30 per week, and does require some flexibility, with a willingness to ensure the smooth running of the business, and must be a car driver.

This role offers the opportunity of a duel role, or overtime if desired to include some community based support on a full or part time basis, to be discussed on interview.

**Responsibilities**
- Coordinate care services for clients, ensuring their needs are met in a timely manner.
- Liaise with healthcare providers to facilitate communication and collaboration regarding client care plans.
- Maintain accurate records of client interactions, services provided, and any changes in care requirements.
- Assist clients and their families in navigating the healthcare system, providing guidance on available resources and support services.
- Monitor client progress and satisfaction, making adjustments to care plans as necessary.
- Conduct regular follow-ups with clients to ensure continuity of care and address any concerns that may arise.
- Participate in team meetings to discuss client cases and share insights for improving service delivery.
- On call responsibilities worked on a rota basis, includes every other weekend with enhanced additional pay per day.

**Experience**
- Previous experience in an office environment is preferred, but not essential as full training will be provided.
- Strong organisational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent communication skills, both verbal and written, with a compassionate approach to client and staff interactions.
- Proficiency in using office software and electronic health record systems is advantageous, but not essential.
- A background in care work, or a related field is desirable but not mandatory.
- Must have some flexibility.

**Job Type**: Part-time

Pay: From £13.50 per hour

Expected hours: 12 - 30 per week
