This is an excellent opportunity for an enthusiastic administrator wishing to join part of the Primary care Team.

Our Recall/Quality team is pivotal to the smooth running of the practice and involves a high level of detail to ensure that our patients are followed up in a timely manner. This includes inviting patients in for routine reviews with clinicians and assisting with the follow up of these reviews in collaboration with our team of clinicians. The post-holder will responsible for the organisation of Long Term Condition reviews, and routine drug monitoring reviews. This will require accurate clinical administration system skills, word processing and data analysis skills, typing skills and excellent written and verbal communication skills. The post holder will work strictly in accordance with practice protocols.

**Job Purpose - Medical Recall Administrator /Diabetic Admin Lead**
- To keep up to date with changes to the Quality Outcomes Framework (QOF), the Primary Care Network Directed Enhanced Services (PCN DES) and various Locally Commissioned services to ensure that the practice are adequately meeting all of the requirements
- Provide a holistic recall service for patients with Long Term Conditions and/or patients who are on prescribed medications.
- Be responsible for the monitoring and co-ordination of the Care Long Term Conditions patient recall system, ensuring that patients with Long Term Conditions are monitored and called in for review in line with Practice policy.
- Conduct searches on the clinical system to identify patients needing medication review and/or investigations.
- Review patient notes, organise appropriate tests and appointments in readiness for Long Term Conditions and medication reviews.
- Work holistically for each patient to ensure that they are called in a minimum number of times for any tests required that relate to all of their conditions and medications.
- Make appointments with patients, explain appointment processes, notify patients of results of tests/investigations and ensure they receive any appropriate information prior to their appointment.
- Ensure patients are followed up and reminded of the needed review and/or investigation.
- Enter all correspondence on to the Practice clinical system using read codes where appropriate and log all invites.
- Establish and maintain spread sheets and databases so that information is logged, easily accessible and secure.
- Assist with the gathering of statistics and information when required, being able to conduct searches.
- Take responsibility for the punctual recall of our diabetic patients.
- Supporting the diabetic nurses with the booking of clinics

**Job Purpose - IT Support for the Practice**
- Be approachable and supportive to provide first point of contact for staff members who need IT support.
- Liaising closely with the CSU in order to maintain IT for the surgery.
- Knowing who to contact to solve IT issues that cannot be resolved within the surgery.
