This challenging role offers the post holder the opportunity to provide high quality patient focused care to frail, older people in Weymouth and Portland. The successful applicant will be expected to engage and act within a range of services delivered in the Hub, including virtual wards, working across inpatient and home environments, rapid access services and care closer to home.

We are especially keen to recruit a practitioner with a varied professional background with the ability to provide advanced clinical assessments and personalised care plans for patients with complex needs. The Community Hubs work as a multidisciplinary model and consist of General Practitioners, ACPs, Allied Health Professionals, Pharmacists, Health and Social Care Coordinators, Adult Social Cars, support self-care, develop appropriate plans, minimise health deterioration via early intervention and work collaboratively with the wider multidisciplinary team to improve outcomes.

This post is full-time, 37.5 hours per week.

The Practitioner will be required to provide proactive and reactive assessments to prevent unnecessary hospital admissions by supporting individuals to remain in their own homes. They will undertake holistic assessments at the point of need and assist patients and their carers to proactively anticipate their future health and care need.An exciting opportunity has arisen for an Advanced Clinical Practitioner (ACP) to join the Community Matron Team/Hub based at Westhaven Community Hospital. The post holder will work together with primary care, secondary care, community services and social services to provide enhanced and pro-active care as part of an integrated frailty team.

Successful applicants must have a broad experience in community and/or urgent care at a senior level. They will be motivated, enthusiastic and have proven experience of working as an autonomous practitioner

You will be required to undertake aPrevention and Management of Violence and Aggressioncourse and it is a condition of your employment that you successfully complete all aspects of this course; if this is not achieved you will not be able to commence working in the post.
