Weymouth & Portland Community Mental Health Team are looking for 1 Mental Health Practitioner to join our well established team in meeting the needs of our demographic. If you are a RMN, OT or Social Worker with experience working with people suffering severe mental illness whether this be inpatient or community based we would love to hear from you.

The care co-ordinator role is for a specified caseload of people with diagnosis of SMI. Building therapeutic relationships with them to enable you to develop recoveryfocused wellbeing plans and implementing them in partnership with them.

The role willalsoinvolve assessments ofpeoplereferred to the service and at times covering the duty role asrequired. The role involvesliaisonwith ourcolleaguesin the Home Treatment Team, Connections, Community Resource Team andPsychiatricliaison as well as the wider community including 3rd sector colleagues and resources.

The post holder will work as part of an established integrated multi-disciplinary team. This team is made up of a several Psychiatrists, ANP, Nurses, Social Workers, Support Time Recovery Workers, Occupational Therapist, Psychologists, Employment specialists and a Carers support worker.

This job is challenging, but if you’re ready for this then we are looking for people who are reliable, committed and motivated with excellent communication, organisational skills and the ability to prioritise. The team are very supportive and you will have case management supervision as well as clinical supervision and opportunities for training.
