Community Mental Health Practitioner

Weymouth & Portland Community mental health Team is looking for a Band 6 Community Mental Health practitioner to join the team. The main roles will be completing assessments and helping to support the duty role within the team.
To carry out initial Community mental health assessments with individuals who are referred to the team.
To liaise with professionals within the team to make a diagnosis and treatment plan for these individual which may include discharge back to referrer.
To have high quality documentation skills, ensuring assessment letters are completed on a timely basis.
To work as part of the duty team on a weekly basis, taking urgent calls and processing referrals.
To provide high quality care and treatment in conjunction with other professionals through the Trust’s agreed multi
- disciplinary assessment and treatment/ care programmes.
