Weymouth and Portland ICRT are looking for a Therapy Assistant Practitioner to work within the Multidisciplinary team, based at Westhaven Hospital. The team consists of OT's, Physiotherapists, Nurses, Advanced Practitioners, Therapy Assistant Practitioners, Rehab Assistants and admin support who work as a team to provide intermediate care and rehabilitation to the adult population of Weymouth and Portland.

We are a friendly and dynamic team, that are committed to providing high quality care to the residents of Weymouth and Portland through integrated and collaborative ways of working in conjunction with our colleagues in Primary and Social Care sectors.

The post holder would be required to provide Holistic assessments and rehabilitation for a range of physical presentations such as stroke and neurology, orthopaedics, frailty, falls, cognitive impairment and many more. Skills required to fulfil this role include knowledge and experience in the community, skills to holistically assess and formulate a patient centred rehab program, knowledge and experience of working with the older person, particularly with frailty.
