We have a great opportunity for a Care Co-ordinator to support our Winsford Branch on a full-time basis working 37.5 hours per week.
As a Care Coordinator, your responsibilities will include Coordinating the rota for care calls, maintain accurate and detailed records, communicating with colleagues and clients regarding their planned visits and supporting the on-call service on a rotational basis.

- Build and maintain positive relationships with our clients, family members, colleagues, and other professionals.

- Ensure the implementation of safe working practices.
