Over the years we have continued to be innovative and responsive and have developed our services to meet the demands of the local mental health landscape. As a result, we have now merged with Cheshire Mind and are excited to be able to build on our coaching service in Cheshire West and develop a new care navigation service in Cheshire East, as we look to widen our geographical reach in order to support even more local people. This new role is a senior position within our growing Cheshire Mind team. We will be working on the Cheshire East Care Navigation Service which aims to make it easier for people with severe mental illness (SMI) to access nonclinical support to foster recovery in local communities. We want to ensure that everyone can access the right support, at the right time, from the right person. The post holder will work with clients presenting with moderate to severe mental illness as they navigate community support to aid their recovery. Clients will often be referred to the service following a hospital discharge. The post holder will lead a team of seven Mental Health Support Workers at Cheshire Mind as well as holding their own small caseload of clients. You will be high profile throughout Cheshire – forging new community contacts and helping us to build our reputation as a high‑quality service provider and partner of choice. Core Duties Lead the teams on both the mental health coaching service and the care navigation service, ensuring cohesion, quality provision and capacity to build for the future. Be instrumental in launching the innovative new Cheshire East service, helping us to create a suite of standard operating procedures, referral pathways and an operational risk register. Provide and oversee a professional one‑to‑one mental health support service in local communities (including face‑to‑face, remote and telephone support, where required) to adult clients presenting with a broad range of moderate to severe mental illness. This will be delivered from local outreach venues across Cheshire East, as well as our mental health hub in Winsford, depending on service need. Facilitate drop‑in and group work sessions when required, as well as managing own caseload of one‑to‑one appointments. Be responsible for the supervision and line management of mental health support workers at Cheshire Mind. Work with the Senior Practitioner to support placement students and volunteers. Meet with referring partners on a regular basis to ensure clear understanding of our referral pathway and criteria. Triage referrals and work with commissioners to ensure appropriate referrals and that clients are able to receive the right support for them. Work to promote the new Cheshire East service and raise awareness with other professionals, GP’s, social workers, social prescribers etc. Act as a community navigator for clients to signpost them to relevant services as appropriate. Work with our partner Smile to map, understand, utilise and cascade a developing community services directory to the team. Liaise with and build relationships with other agencies and the wider support network around the clients. Be the Designated Safeguarding Lead for the service (training will be provided). Follow safeguarding procedures at all times and ensure the relevant support/referral pathways are actioned for the client. Work flexibly to allow us to offer the best service possible to all clients – this may involve working one evening per week. Produce relevant reports on service performance, as requested by the Head of Services and/or commissioners. Appropriately manage any concerns raised by a service user, to ensure the clients are supported adequately. Be responsible for one’s own safety and well‑being, as well as that of service users and other staff – following health and safety and lobe working procedures on site. Work as an integrated part of a team to ensure the smooth running of Cheshire Mind services. Be willing to attend CPD on a regular basis and attend service‑specific training as required. Work in line with North Staffs (and Cheshire) Mind’s current practices, policies, and values, including confidentiality and safeguarding. Demonstrate that you are working in accordance with the BACP Ethical Framework at all times. This document can be found on bacp.co.uk in the ‘Events and Resources’ section. Be prepared to work in a variety of ways across a range of internal services. Work in a time limited, but appropriate way with clients to help them identify, understand, and resolve or manage the issues which have brought them to our services. Maintain up to date knowledge of issues which affect the mental health of individuals, families, and communities. Maintain knowledge base by attending relevant training and undertaking relevant reading. Use treatment evaluation measures (Recovery Star, PHQ/GAD etc.) as required in the service. Ensure client data and session notes are recorded appropriately and in line with GDPR on CRM (Charity Log) as appropriate. Ensure all relevant information is communicated to administration staff, where applicable. Undertake your own clinical supervision of 1.5 minimum hours per month. Attend staff meetings, team meetings, training etc. as required by North Staffs Mind. Undertake any duties as required by North Staffs Mind to maintain an efficient and client friendly service. Please note, a driving licence and use of your own vehicle is essential for this role. An enhanced DBS check will be completed for the successful applicant.
