Tezlom are assisting their client in the recruitment of an experienced Care Coordinator to join their team at a well established branch, based in Winsford. This position will see you liaise with staff, clients and external healthcare professionals to ensure the seamless delivery of organized and effective care whilst coordinating a team of care workers.

Main responsibilities within this role include:

- Line manage and supervise Care Workers
- Organise and coordinate weekly, rotas and care services
- Maintain full compliance in line with CQC regulations and Framework requirement, includes quality monitoring checks, file audits, monitoring staff performance
- Update and evaluate Care plans and ongoing needs of clients
- Develop and maintain relationships with clients and care staff team
- Liaise with external healthcare professionals and organisations to ensuring the delivery of person-centred care
- Carry out on-call coordinating duties
- Assisting in recruiting and training
- Staff supervisions and spot checks and reviews as required
This position is office based Monday-Friday and does require On-call shared by a team of 4 coordinators. Previous experience of at least 3 years in the care sector is essential and 2 years previous experience in the same role is required to be considered for this position. Salary is 26-28k plus on call.

**Requirements**:

- Minimum one year experience of Care Coordinator required.
- Driver with access to own vehicle.
- Fully Vaccinated against Covid-19.

**Job Types**: Full-time, Permanent

**Salary**: £26,000.00-£28,000.00 per year
