We have an opening for you to join us as a Care & Support Team Leader at our Croft Community in Malton. As a Team Leader you will ensure that all needs assessments, support plans and risk assessments are kept up to date. There will also be direct engagement and liaison with social workers, family members and key workers as required. You will be responsible for ensuring that care and support is of a high standard, focused on person-centred outcomes and delivered within budget.

**Duties & Responsibilities**

**Main responsibilities**:

- Encourage and support people to maintain a clean, tidy and welcoming living environment.
- Support people to use appropriate laundry equipment enabling them to do their laundry and look after their clothing so they have suitable and clean clothing to wear.
- Plan, shop and prepare healthy and wholesome meals alongside the people you support, considering their choices and wishes.
- Support to maintain health and well-being by assisting people to arrange and attend medical appointments.
- Support and encourage those you support to maintain an active and supportive social life considering individual choices and wishes.

**Person Specification**:

- Level 2 Diploma Health and Social Care (Level 3 desirable)
- Driving licence desirable but not essential if you live locally.
- A year at team leader level or supervisory experience (desirable) or very experienced support worker (at least 2 years).
