**Salary**: £25,000-£26,906 Salary dependent on skills, experience and knowledge.

**What will you do?**

Your role will be to engage with parent/carers who require time-limited support with their mental health and wellbeing needs. As part of the multi-disciplinary Family Solution Plus team you will promote early engagement and aim to de-escalate families in terms of social care intervention.

The AFP will take a holistic and person-centred approach to supporting adults to identify their strengths, set goals and support motivation to improve their mental health and wellbeing through a variety of interventions. By working alongside social care, substance misuse and domestic abuse colleagues you will also offer support to social workers and other professionals and be available for consultation and the mutual sharing of good practice between professionals.

Adult Facing Practitioners will provide one to one sessions to help and support parents with mental health issues that are impacting on their wellbeing and parenting capacity, working through a series of tools with parents to provide person centred, strategies and interventions.

**Key responsibilities**
- Support service users to improve or maintain their mental health and wellbeing by offering signposting support.
- Maintain accurate information on support and services available in Oxfordshire.
- Collect and accurately record service user data onto a secure database in a timely manner.
- Liaise with colleagues, partner organisations and professional agencies to support service user needs.
- Keep up to date with the various services offered by NHS and third sector providers to enable appropriate information and signposting.
- Conduct regular dynamic risk assessment and take appropriate and timely actions to manage the risks in relation to staff, service users and buildings.
- Undertake assessments of parents/carers mental health as part of home visits and appointments in the community.
- Maintain knowledge of Adverse Childhood Experiences (ACE’s) and the identification and possible impact these could have on parenting capacity and mental health.
- Maintain effective professional relationships with the FSP framework staff.
