We are looking to recruit a Clinical Lead to join Chester South Community Care Team in the Chester area. You will be joining a multidisciplinary team including but not limited to; team manager, enhanced clinical practitioner, clinical leads, community nurses, AHP's and many other specialist professionals who visit the team.
We are looking for someone who is enthusiastic, self-motivated and who provides confident support and leadership and coaching to clients and clinicians. You must excellent communication skills and an ability to work in partnership with clients and their families/carers and when necessary, act to safeguard individuals who are at risk from harm and abuse.
This role is Fixed term until 28/06/2026.
- Provide a highly visible clinical profile with leadership and clinical oversight of a designated Care Community caseload
- Working collaboratively with the wider health, social care and the third sector partner agencies to meet the needs of patients
- Ensure patients receive the right care, by the right person at the right time and the right place
- Placing the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with mínimal avoidable delays, and an emphasis on people achieving independence whilst improving quality of care
- To provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives
- To be aware of local public health community profiles and how this influences the required care for the population
- According to community profiles: understand the skills required from the Community Care Team
- To focus on the delivery of person centred care
- To provide effective, evidence based care to a defined population through case load management and monitoring of care standards
- Provide clinical leadership, coaching and development of the clinical team.

Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.
