Nantwich Rural Primary Care Network (PCN) and CWP’s Children Young People and Families mental health services have worked together to enhance access to mental health support.
We are looking for an experienced Mental Health Practitioner who is interested in working as part of the GP Multidisciplinary Team in Nantwich Rural Primary Care Network. This is part of an exciting new development to improve collaborative working across primary and secondary care, the role will contribute to the expansion of community mental health services as set out in the NHS Long Term Plan. This brand-new role has arisen in Nantwich Rural as part of the transformation with our partner agencies to develop the interventions that can be offered at primary care level. The successful applicant will be part of a team of practitioners aligned to our Cheshire Well Being Hub working in identified GP practices across Nantwich Rural.
This role will have strong links into the local services and voluntary sector. We envision a seamless path into this team which can also help support the young person as well as parents and families
The role will include completing assessments, care planning and delivering interventions for patients registered at the GP practice who have mental health needs. The role will also involve a strong navigator element to ensure the provision of advice, education and support to children, young people or family members. You will also act as the interface between partner agencies to ensure effective transition to primary care and referral into secondary care settings when necessary. You will also be actively involved in the interventions provided by the wider GP multidisciplinary team and provide expert advice and support to enable them to provide an effective service for children and young people with mental health needs including their physical health needs.
You will receive direct support and guidance from the GP's about your workload but also be line managed and professionally supervised by the CWP Band 7 Cheshire Wellbeing Hub Lead. As the post is hosted via CWP you will also have access to training, education and support from CWP. This new post would suit individuals who are committed to collaborative working to improve outcomes for people with mental health issues.

Primary Care Networks (PCNs) are groups of GP practices working together in a local geographical area. Nantwich Rural Primary Care Network is a collaboration of the 5 GP Practices within the town who collectively care for over 30,746 patients.
