You will be a main point of contact for service users and carers in the team and will be required to monitor their mental state, progress and level of functioning and requesting feedback from families (training provided) to help identify early signs of transition to psychosis whilst delivering a range of recovery-focussed interventions. The post holder will be expected to feedback clinical impressions of service user activity an contact to the team as soon as possible to ensure the team are kept up to date and ongoing care can be reviewed as required.
The post holder will react pro-actively in cases of service user crisis and/or distress and ensure escalation of such matters to a registered professional to put in place immediate plans to resolve the situation as appropriate.
