To actively monitor the departments overdue follow up lists, prioritising patients appointment dates and working towards clearing the waiting lists To work closely with the Service Manager, Clinical Office Team Leaders, Paediatric Appointments Team, Clinical Office Staff and Outpatient teams, Clinical staff and nursing staff within the division to ensure patients are actively managed through patient pathways. To ensure that appropriate requests for investigations are monitored following the clinicians If diagnostic delays occur or appear imminent liaise with the relevant service for action. To monitor patients pathways from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process and report to clinical offices any discrepancies. To liaise with members of the management teams to ensure coordination of all elements of patient This may include highlighting patient target dates etc.

To ensure all information relating to the patients pathway clearly identifies all relevant target dates to aid other users of the relevant systems, such as Cerner in allocating resources appropriately. To support the division in implementation of robust follow-up tracking, ensuring no patient is ever lost in the system. To track and validate follow-up activity as per the following scenarios: Overdue follow up activity with missing/delayed Disposal codes denoting a follow-up required but this was not Routine RTT validation of follow-up activity To work with clinical offices in developing, implementing and monitoring changes to the patient pathway tracking/validating process and supporting improvement activities as To maintain up to date knowledge of all standards, targets and processes relating to the patient pathway process and to be a point of reference for administrative teams across the To provide, local training and support to other staff within the Trust regarding Referral to Treatment rules, processes and procedures.

To adhere to the Trusts Joint Access Policy and related national RTT To be responsible for sending out relevant appointment letters, and cancelling/rebooking patients appointments. Responsible for checking all clinic letters are completed accurately and outcomes actioned and recorded on Surrey Safe Care / Cerner. Manage changes in clinic templates as directed by Team Leader/Service Manager.
