The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care. The post holder will provide a single point of contact for the management of bed availability and allocation within ward. The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and ‘real time’ understanding of the ward’s current and predicted capacity position and the timely and well planned discharge process.
- To support the ward manager and ward co-ordinator in maintaining patient flow
- To manage the maintenance of e-track to ensure it reflects the current
- discharge plans and the ward bed availability
- Liaise with the duty manager when a bed is available.
- Attend Whiteboard rounds, identify actions required and actively manage
- agreed discharge plans for patients with predicted discharge dates within
- the next 3 days.
- Maintain whiteboards and e-track information to ensure it is up to date.
- Arrange or where necessary photocopy notes for patient transfer.
- Assist in the completion of discharge check lists to ensure accurate
- discharge planning is evident.
- Chase doctors for completion of discharge letter and deliver TTOs if
- necessary.
- Ensure diagnostic requests are expedited promptly and escalate any
- delays to the appropriate person.
