Job Summary
- To provide clinical leadership, support and supervision to clinical To support an identified team of practitioners with advice and guidance on the management of complex clinical case.
- To work as an integral part of the CMHT which will include carrying a small complex
- To participate in the extended working hours as required by the needs of the service
- To provide clinical leadership and structured clinical supervision to the team.

1. Maintain consistency and quality of care for all service users under the care of the CMHT andsupport effective and timely discharge to Primary Care
2. Support safe ,efficient and effective high quality transitions in and out of the CMHT
3. Ensure the team works in a collaborative manner with other PCFT services, primary care and other partners
4. Support the team in delivering recovery focused mental healthcare
5. support the team in delivering on the physical healthcare agenda
- To ensure that clinical standards are, monitored and evaluated, and ensure both they and the team provide care of the highest quality to patients
- To liaise closely with social care leads, housing associations, IPS workers and other agencies to promote social support and aid recovery and promote this way of working within the team
- To liaise closely with, general practitioners, to ensure clinical pathways are supportive of patient flow from CMHT’s to Primary Care and vice

Main

**Responsibilities**:
Managerial
- provide case management supervision to the staff team, promoting stepping down services to care support and the timely discharge back to Primary Care
- To analyze, validate and minimize any internal pathway blockages such as Care Co-coordinator & Psychology waiting lists.
- To be an effective decision-making and clinical resource to support the Team manager in the selections and recruitment of staff, and to develop and promote strategies aimed at staff retention
- To support the Team manager to complete appraisals for relevant allocated staff within the team.
- To support the team in developing and building effective communication with all other services.
- To take appropriate action with regard to complaints and reportable incidents within the service as directed by the Team Manager.
- To contribute to the implementation of local and national objectives.

Clinical Responsibilities
- To provide guidance and support to clinical staff in the management of complex cases.
- To provide guidance and support on MHA, COP/DOLS, Best Interest.
- To be responsible for the development and maintenance of effective clinical pathways between CMHT and other services.
- To provide additional leadership and guidance to other team members in risk assessment, risk management and positive problem solving and lead on debriefs following incidents.
- Develop confidence and competence within the team for managing complex clinical cases and issues.
- To take a lead role in the provision of clinical leadership and structured clinical supervision to the team.
- To lead on and ensure that the team undertakes appropriate and in-depth assessments and on-going intervention, and undertake the duties and responsibilities of a lead worker/CPA co-ordinator and maintain an active lead worker caseload in line with CPA principles.
- To work collaboratively with medical staff/GP’s to produce clinical management plans.
- To ensure the provision of the highest possible standard of care within the CMHT and ensure the team follow these principles Ensure patients within the team have an up to date integrated care record, reflective of their risk and care needs in line with policy and procedures.
- To assist people to achieve maximum independence through the use of care programmes, which are culturally sensitive, individualized, safe and consistent with accepted practice and ensure the team also follow these principles
- To participate and lead in mental health education and provide advice/support as necessary to patients and their carer’s on the maintenance of good health and prevention of relapse and ensure the team also follow these principles
- To receive formal managerial supervision from the Team Manager and clinical supervision from a suitable expert.
