Here in Teesside, Durham and Sunderland Home Group have a large number of support services, which help our many customers who all have individual and varying needs. Due to the expansion of our team we have a fantastic opportunity for an enthusiastic Support Coordinator like you, to join our Central Resource Team (CRT).
Instead of working in one service, CRT gives you the opportunity to work across a wide range of supported services and alongside our specialised teams helping to enrich our customers lives.
As our Support Coordinator, you’ll help change our customer’s lives for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create individual and personalised support plans with and for customers. Each plan will look to meet the customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.
You’ll also have responsibility for coordinating our support workers and working collaboratively with external agencies, to assess and engage support for our customers.

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe) You’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system. This means that you don’t have to be in the office as you can complete support plans on the go.
As this is a community based job, you need to be able to drive, as well as having access to a vehicle insured for business purposes. Don’t worry, we’ll pay you expenses to cover this use
- When you start your job, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._
**Working hours**
You’ll work between the hours of 09:00 - 17:00, Monday to Friday. We aim to be flexible in our approach, to meet the needs of our customer’s but also to ensure our colleagues have a good work - life balance.

Our team is an outgoing, diverse, and supportive group of individuals, who are always willing to lend a helping hand.
We are led by our amazing Senior Client Service Manager, Yvonne who has worked for Home Group since 2014. She started as a Support Worker and took full advantage of all the training and experience we offer here at Home Group.
Supporting Yvonne are our wonderful Client Service Managers Shannon, Libby, Sam and Tom who oversee the team on a daily basis. They’re perfectly placed to support you as you join.
