Consultant in Community Medicine for Older People

We are looking for a substantive Consultant in Medicine for Older People working for Solent Adult Services Southampton.
Here at Solent NHS Trust, our Adult Services in Southampton provide community care for Southampton Clinical Commissioning Group. This comprises of inpatient and community based rehabilitation services, medical support for community nursing and urgent response services and Comprehensive Geriatric Assessment at home for people unable to access normal outpatient clinic services.
Although predominantly a service for older people, it is defined more by frailty or rehabilitation needs and there are no age restrictions. Community-based care for older people is a growing subspecialty of Geriatric medicine and political reforms will drive more care into community settings. In Southampton, there are exciting opportunities and developments as part of the H&IoW ICB and Southampton City Council Ageing Well Strategy. This includes redevelopment of the Western Community Hospital site to provide new inpatient rehabilitation facilities.
There is a close working relationship with clinicians at University Hospital Southampton and Solent have in-reach teams driving early and safe discharges from the Emergency Department, Same Day Emergency Care and Acute Medical Units; and pulling suitable patients through to the rehabilitation wards. There is likely to be some rotation with other Solent-employed consultants to deliver 2 sessions weekly on Same Day Emergency Care Unit.
