Haslemere Homecare Ltd are a family owned and run domiciliary care provider serving Surrey, Hampshire and West Sussex. We are recruiting a Client Coordinator to join our busy team.

Our Client Coordinators are the first point of contact for any potential new customer. Your role is to introduce the company and create tailor made care plans and risk assessments to ensure the customers health and social care needs are being catered for. Your role will involve working with third parties such as Gp's, Social Workers and other health professionals' to ensure we are giving the customer the best possible care.

About you:
Experience in domiciliary care is essential

A full driving license is essential.

Excellent interpersonal skills.

Ability to understand and follow policies and procedures.

Reliable and honest with good time keeping skills.

Good English skills both verbally and written.

Enhanced DBS required paid for by the company.

Experience in Care Planning and Risk Assessments.

Experience with managing the On Call Phone.

A NVQ level 2 or equivalent in Health and social care is desirable.

About the job and what we would expect from you:
To be the first point of contact for all new care enquiries.

To meet and assess all potential and new clients.

To produce care plans and risk assessments using the Pass system, updating as required.

To complete customer reviews and update care plans accordingly.

To maintain skills at a current level and undertake such training and development as may from time-to-time be required to maintain practices as up to date.

To liaise with families and third parties to ensure the best quality care is provided.

To work with Clients to ensure they are happy with the care being provided.

To take share of the on-call phone, being a point of contact for emergency situations and cancellations. ( 1 week in 6).

To be able to report and record information effectively onto the system used.

To be able to work effectively on your own and as part of the team.

To provide personal care to our service users as required.

This is a full time salaried position with the opportunity for career progression.

Competitive Salary £20,000 -£25,000 Depending on experience.
