An exciting opportunity has arisen to join the Sherborne Integrated Community Rehabilitation Team based at the Yeatman Hospital in Sherborne.

The Sherborne Integrated Community Rehabilitation Team is a multi-disciplinary team offering therapy to a wide range of people within their home environments to prevent avoidable hospital admissions and facilitate earlier hospital discharge as well as in-reaching onto the wards.

It is essential that you have experience of rehabilitation or a caring role, preferably in both ward and community settings, as well as having good verbal, written and team working skills. You will need to be flexible in your approach and confident to work alone. Basic IT skills and NVQ level 2/3 would be advantageous.

We are looking for a hard-working and dedicated individual to contribute to the teams role in providing clinical rehabilitation and intermediate care to both community and ward patients under the direction of qualified practitioners.

You will need to be a positive, compassionate and an excellent communicator

This is a full-time post, 37.50 hours per week - hours to include some weekends and Bank Holidays.

We are a small, passionate and dedicated team of Occupational Therapists, Physiotherapists and Rehab Assistants who strive to deliver the highest standard of care for our patients.

The role will involve working both in the community and on the wards of the Yeatman Hospital.

In return for your enthusiasm and commitment, we can provide an opportunity for you to develop your clinical skills and experience in a friendly and supportive team, with regular supervision and appraisals.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check.

To contribute to the teams' role in providing clinical rehabilitation and intermediate care to both community and ward patients under the direction of qualified practitioners.

To assist in the provision of administrative support to the team.
