This is an exciting opportunity for a Care Co-ordinator to work across the neighbourhood services covering West Cheshire

You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside team and partners including GPs and the voluntary sector, to support patients within the care community.

This post is 12 months Fixed term/Secondment, full time 37.5 hours per week, Monday To Friday 9-5pm.

This role will include the following
- Good interpersonal and communication skills.

Co-ordination of multi disciplinary meetings.

Management of of daily meetings.

Liaising with multi-disciplinary team members.

Signposting team members and patients/carers to relevant services.
