As a **Patient Aftercare Coordinator**, you will play a vital role in ensuring patients receive the highest level of care post-treatment. You will be responsible for managing the recovery process, providing support and guidance, and helping patients navigate their aftercare journey smoothly. Your role will involve maintaining consistent communication with patients, offering educational resources, and collaborating closely with our clinicians and nurses to ensure optimal outcomes.
**Responsibilities**
- Serve as the primary point of contact for patients, addressing inquiries and providing information about services.
- Schedule and confirm patient appointments, ensuring efficient use of resources.
- Maintain accurate patient records and manage documentation in compliance with privacy regulations.
- Collaborate with Clinicians to coordinate patient care plans and follow-up appointments.
- Handle billing inquiries and assist patients with understanding their insurance benefits.
- Provide support during patient visits, ensuring a welcoming and comfortable environment.
- Participate in quality improvement initiatives to enhance service delivery.
**Requirements**:
- Proven experience in an administrative or office role, preferably within a healthcare setting.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Excellent communication skills, both verbal and written, with a focus on customer service.
- Proficiency in using office software and electronic health record systems.
- Ability to work collaboratively within a team while also being self-motivated.
- A compassionate attitude towards patients and their families, demonstrating empathy and understanding.
- Attention to detail and a commitment to maintaining confidentiality.

The role is full time
7 days a week coverage
2 shifts - 8AM to 4PM and 2PM to 10PM
Weekends on Rota
40 hours per week
Location: Warrington
**Job Types**: Full-time, Permanent
Pay: £28,000.00-£32,000.00 per year
