**Job Overview**

We are seeking to appoint an enthusiastic Community Hub Coordinator to support our teams and service users, within Community Health Services for Nursing and Therapy based at Braunstone Health and Social Care Centre in the Community Nursing office (free parking is available and this base is close to public transport). You will need to demonstrate good communication skills and be able to work independently in a variety of community settings. You will need a flexible approach and the ability to work as part of a team and to use your own initiative.

The following skills are essential to the role:

- Attention to detail.
- The ability to multi-task.
- Excellent telephone manner and communication skills.

**Job Description Summary (Main duties and responsibilities)**

To include but not limited to
- General administrative duties.
- Using the electronic patient record system (SystmOne).
- Completing electronic referrals and tasks.
- Assisting where appropriate with data collection for audit purposes.
- Data entry.
- Supporting Core Community Nursing and Therapy staff, Team Leads and clinicians.
- Ordering and monitoring of equipment and stores.

You will need to be a good team player, with excellent organisational and communication skills.
