**Role-specific Duties**:
The Care Coordinator’s responsibilities include but are not limited to the following:

- Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing
- To supervise staff and ensure that all staff contribute to the efficient running of the service
- To organise and coordinate care delivery in accordance with current best practices, according to policy and procedures, agreed with standards, legislative requirements and relevant regulations under the direction of the Manager and within the financial plans
- Ensure that the care service is delivered to service users at the times and for the duration specified in the relevant individual agreement(s) and contract standards
- Be responsible for promoting and protecting the welfare of those individuals supported by the service
- Develop effective working relationships with other employees within the service Work in cooperation with members of the multidisciplinary teams to maximise opportunities for service users
- Identify areas for business growth and recruitment needs
- Attend statutory training and any other training as directed by management

**What are we looking for?**

We are looking for a dynamic and caring individual with experience of Care Coordination who is also big on promoting independence and ensuring that every task is driven by the customer's own choice and completed with them, not for them.

You could be helping people who have mental health problems, learning difficulties, physical disabilities, or people who would just like a bit of help in general. We support people from the ages of 18 years old right through to retirement and beyond.

**What hands on work will you do?**

Each day is varied, and flexibility is required.

You will be coordinating care and support workers whose schedule is tailored to the needs and wishes of the customer. Through getting to know the people we support; you will identify and work with them to achieve their goals and help enable them to have choice and control over their lives.

You will also be expected to carry out everyday tasks which can include general administration, on-call support (on a rota system) and on occasion be part of the contingency.

Your role will be someone who goes above and beyond what would be expected from generic home help service and someone who always behaves professionally with integrity and with honesty.

**Requirements**:

- To be an honest, empathetic, hardworking and trustworthy individual
- Experienced in at least 1 year in Coordination
- Good knowledge on CQC and person centred approach
- A full driving licence, access to a vehicle and business insurance is required
- You will be trustworthy and honest.
- You will have a caring approach

**Job Types**: Full-time, Permanent

**Salary**: £20,000.00 per year
