Job Summary We are looking for a new Care Coordinator to join our successful EHCH team. The candidate will have excellent communication and administrative skills to support our Ageing Well team working on the Enhanced Health in Care Homes and Housebound service. About Us IMP Healthcare is a Primary Care Network (PCN) with 9 member GP practices in the Lincolnshire areas. We provide services to over 70,000 patients, including Mental Health, Social Prescribing, Enhanced Health in Care Homes, Housebound services, clinical pharmacy, first contact physiotherapy, and an Extended Access service, enabling appointments outside core GP hours. Identify and manage a caseload of patients with changing needs, ensuring appropriate support is available. Support people to develop, review and update personalised care plans, communicating with GPs and other professionals via online care records. Make and manage appointments related to primary care. Assist patients in transitioning between secondary and community care, including follow‑up appointments. Refer onwards to social prescribing link workers and other services where appropriate. Liaise regularly with multidisciplinary professionals, facilitating coordinated care. Participate actively in PCN multidisciplinary team meetings. Identify opportunities for service improvement and provide feedback to support business planning. Contribute to the development of policies on equality, diversity and health inequality reduction. Adhere to organisational policies, confidentiality, safeguarding, lone working, information governance, and health and safety. Engage in ongoing professional development, supervision, and line‑manager discussions to achieve targets. Act as a champion for personalised care and shared decision making within the PCN. Person Specification Qualifications Essential: Good standard of secondary education, including English and Maths. Desirable: Healthcare qualification. Experience Essential: Experience working in a healthcare setting or a public‑facing role with excellent customer service skills. Desirable: Primary Care / General Practice experience; good IT and computer skills.
