Community Hospital Discharge Navigator Band 3

Band 3 Community Hospital Discharge Navigator 37.5 hours per week

We are looking for a dedicated, confident individual, to work in our community hospital discharge team. You will be expected to work in different community hospital sites to support the Multidisciplinary team, in ensuring safe and effective discharges for our patients.

Working 7.5hrs a day, including some weekends and Bank Hollidays.

The discharge navigators role is to help ensure safe and effective discharges in line with the current government discharge guidelines.

The post will include working remotely over six community hospitals, based around the north of Cumbria. Working with The MDT, patients and relatives to organize, structure and enable patients discharges, following hospital and Government policies and guidelines.

Good interpersonal skills, organization and IT knowledge are all beneficial for this role.

You will be an effective independent worker who equally works well within a team environment.
