Band 3 Health Care Support Worker - All Age Access Team, Harplands Hospital:
We are looking for enthusiastic, forward thinking, passionate and motivated individual to join our OUTSTANDING All Age Access Team at the Crisis Care Centre, Harplands Hospital. If you are ambitious, innovative and want to make a difference, we want to hear from you.

Our All Age Access Team is part of the Crisis Care Centre, at Harplands Hospital, Stoke-on-Trent which has been designed to the highest level of specification. This modern purpose designed unit is the central point for all those in need of a Crisis Assessment/Intervention. Teams within the Crisis Care Centre include the All Age Access Team, Home Treatment Team, High Volume Users Team and a commissioned place of safety for those detained on a section 136.

To provide support, and spend therapeutic time with an allocated group of service users.
- To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.
- To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualized care plans.
- To provide support and day-to-day supervision to junior staff members, demonstrating the Health Care Support Worker role and role modelling good practice.

1. Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
2. Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team. (query blending the above two - first is about patients/public, second is healthcare staff)
3. Relates appropriately with service-users, carer's, colleagues and others - at all times maintaining professional boundaries.
4. At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
5. Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
6. At all times minimizes risks to self and others by undertaking safe working practices.
7. At all times works within agreed and established Trust policy and operational procedures.
8. Works with other Trust services to promote seamless service-user person centred care from referral/admission, to transfer and discharge on identified care pathways.
9. Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
10. Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff. Responsible for regularly working with service-users to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.
